Where to Live Other than Home
Most people want to live at home. Some people find licensed care settings to be a better option. NorthWest Senior and Disability Services (NWSDS) has public files for these settings in Clatsop, Marion, Polk, Tillamook, and Yamhill counties. The files have information about licensing, monitoring, and any reports of abuse. To view them, please contact your local NWSDS office.
Adult Foster Homes
Adult foster homes are private homes with family-style living. They offer room, board, and physical care for up to five people. NWSDS licenses, inspects, and monitors these homes in Clatsop, Marion, Polk, Tillamook, and Yamhill counties. NWSDS offers master lists of Adult Foster Homes in the five county area and lists of vacancies by county.

Assisted Living Facilities
Assisted living facilities have private apartments with a bathroom and a small kitchen. They offer dining room meals, housekeeping, physical care, and call systems for 24-hour care. They are licensed and regulated by the Department of Human Services, Aging and People with Disabilities.
Residential Care Facilities
Residential care facilities are group settings. They offer room, board, supervision, and physical care for six or more people. They offer either a shared or private room. They do not have to offer private bathrooms or kitchens. They are licensed and regulated by the Department of Human Services, Aging and People with Disabilities.
Nursing facilities provide nursing care 24 hours a day in a medical setting. They offer services such as on-site physical rehabilitation, recovery after being in the hospital for a serious illness or injury, and restorative services. In Oregon, all people must be screened before entering a nursing facility. This is called a pre-admission screening. The screening is free and helps to make sure a nursing facility is the right fit for care needs. Nursing facilities are licensed and regulated by the Department of Human Services, Aging and People with Disabilities.
Facilities with an Endorsed Memory Care Community
Some facilities offer an endorsed Memory Care Community. They offer residents with Alzheimer's disease or dementia a positive quality of life, protection, and increased individualized care to promote rights, dignity, comfort, and independence in the least restrictive setting.
